A micro-ecological approach to home care for AIDS patients

This article attempts to reformulate some of the conceptual links between the health status and health behaviour of individuals on the one hand and the domestic production of health care on the other. Reformulating these linkages results in – what I call – a 'micro-ecological approach to health'. The approach will be applied to the subject of home care for AIDS patients, particularly in Sub-Saharan Africa. The attempt at formulating such an approach is motivated by the idea that traditional disciplinary boundaries, such as those between medical anthropology, medical sociology, public health and nutrition sciences, not only stand in the way of understanding the processes involved, but also obstruct finding comprehensive solutions to major health problems.
The article begins with a theoretical discussion of the context of households and care in order to delineate the micro-ecological context. Subsequently, two theoretical frameworks are critically examined for their relevance as building blocks for the new approach: the framework of the household production of health (HHPH) and the critical ecological model for medical anthropology (CEMMA). Based on these discussions the 'micro-ecological approach to health' (MEAH) is formulated and applied to home care for AIDS patients. In dit artikel wordt een conceptueel kader ontwikkeld om de relaties tussen de gezondheid van individuen enerzijds en de huishoudelijke productie van zorg en gezondheid anderzijds te onderzoeken. De ontwikkeling van een dergelijk kader, een 'micro-ecologische benadering van gezondheid', wordt gemotiveerd door het belang dat ik hecht aan het doorbreken van de scheidslijnen tussen de verschillende disciplines die zich met gezondheid van de mens bezighouden. In het eerste deel van het artikel worden de conceptuele lijnen uitgezet, gebruikmakend van bestaande theoretische benaderingen van huishoudelijke productie, zorg, en gezondheid. In het tweede deel van het artikel wordt de micro-ecologische benadering toegepast op de thuiszorg voor AIDS patiënten

Why should households care? A rejoinder

This rejoinder addresses the comments on the article “A micro-ecological approach to home care for AIDS patients”. The key issue of my reply is the rationale for choosing the household as a level of analysis in the MEAH framework, since critical questions were asked about this in the comments. Furthermore, the rejoinder discusses the methodological implications of applying the model and attempts to provide further clarification on the significance of external linkages for households and the concepts of vulnerability and coping.

Ziekteverhalen van TB patiënten
Omgaan met de broosheid van het bestaan

Chronische en levensbedreigende zieken, zoals TB in het verleden, bedreigen de identiteit en het bestaan. Men ziet zich geconfronteerd met onzekerheden over de afloop van de ziekte en toekomstplannen vallen in duigen. Het vraagt een grote flexibiliteit om met deze onzekere situatie om te gaan. In lineaire ziekteverhalen gaan patiënten uit van een lang gezond leven. Achterliggend idee is dat het leven en de toekomst beheersbaar zijn. Op grond van een kwalitatieve studie van ervaringen van oud-TB patiënten laat dit artikel zien dat polyfone ziekteverhalen in vergelijking met lineaire verhalen meer houvast bieden in het omgaan met de broosheid van het bestaan. In dit artikel worden de relevantie, uitgangspunten en gevolgde narratieve werkwijze gepresenteerd. Dan volgt een beschrijving van de persoonlijke ervaringen van oud-patiënten, en de leidende thema's in hun verhalen. Deze worden vervolgens gerelateerd aan inzichten op het terrein van 'illness narratives'.

Illness narratives of TB patients: Coping with the fragility of life

Just after the Second WorldWar, life threatening illnesses, such as TB, also threatened people's identity and life. Plans for the future were shattered, uncertainties prevailed. Linear illness narratives colonise the future, assuming that it can be controlled. Drawing on a qualitative study of the stories of people with tuberculosis this article shows that polyphonic illness narratives provide a better guidance to interpret people'sway of coping with the fragility of life. The article sets out to present the relevance and assumptions of a narrative approach. This is followed by a description of the experiences of the patients and the themes in their stories. The themes are then related to a framework on illness narratives.

Onderzoekservaringen van gezondheidspersoneel in Bolivia

Toegankelijkheid van zorg staat centraal in Bolivia, een land dat te kampen heeft met hoge moeder- en kindsterftecijfers. De laatste regeringen stelden beleid op waarin nadruk wordt gelegd op gender en interculturaliteit. Onderzoeken zijn uitgevoerd waarin dezelfde conclusies terugkeren: om de bevolking meer gebruik te laten maken van de gezondheidsdiensten in het land, moet de toegankelijkheid verhoogd worden door economische en sociaal-culturele barrières weg te nemen. Waar onderzoek tot doel heeft data te verzamelen over de bevolking met betrekking tot gezondheidszorg, strekken de conclusies van de rapporten zich uit tot aanbevelingen aan de gezondheidswerkers. Het ontbreekt hun echter vaak aan een patiëntgerichte houding waarin de nadruk gelegd wordt op interculturele communicatie en respect voor de andere (inheemse) cultuur. Het hieronder beschreven onderzoek was erop gericht het ziekenhuispersoneel in het onderzoek te betrekken om het luisteren, een belangrijk aspect in de kwaliteit van zorg, te bevorderen. Het eerste deel van dit artikel bespreekt de methodologische veranderingen toegepast in het onderzoek. Het tweede deel richt zich op de vorming van de zorgverleners door hun opleiding.

Towards a better dialogue: Research experiences of health workers in Bolivia

Much attention is paid to the accessibility of health care in Bolivia, a country dealing with high mother and child mortality rates. Research on the accessibility of health services has come to the conclusion that it needs to be increased by reducing economic and social-cultural barriers. Where research aims to gather data regarding the population's views on health care, the conclusions of the reports generally include recommendations directed at the health workers. They often lack the patients' perspective emphasizing intercultural communication and respect for the other (indigenous) culture. The research described in this article was aimed at involving the hospital health workers in the research to stimulate listening, an important aspect of quality of care. The first part of this article concentrates on the methodological changes applied in the research. The second part focuses on the role of education in shaping health care workers' professional attitudes.

Staarsteken in Tsjaad
Verkennend onderzoek naar traditionele behandeling van oogklachten in Noordoost Tsjaad

Traditionele genezers behandelen oogklachten en staar in Tsjaad. Uit een verkennend onderzoek onder traditionele genezers, blinden en oogpatiënten blijkt dat lokale medicijnen en de staarsteek desastreuze gevolgen hebben. Staarstekers praktiseren in het Noordoosten van Tsjaad, waar mensen nauwelijks toegang hebben tot de reguliere gezondheidszorg. Van de 1960 onderzochte patiënten zijn er 224 blind aan tenminste één oog door traditionele behandeling. Staarsteken veroorzaakt in de meeste gevallen blindheid. Minder dan vijf procent van de behandelde mensen heeft een gezichtsvermogen van drie-zestigste of meer met brilcorrectie. Traditionele genezers kunnen ongehinderd hun gang gaan door onwetendheid van de bevolking, hun angst voor de westerse geneeskunde, de grote afstanden tot gezondheidsposten en het ontbreken van overheidstoezicht. Voorlichting en permanente reguliere oogzorg zouden een halt toe kunnen roepen aan deze praktijken.

Couching: Exploring traditional treatment of eye diseases and cataract in the Northeast of Chad

Due to local medicine and couching (traditional treatment of eye diseases)traditional healers in Chad treat eye diseases and cataracts with disastrous effects.. In the Northeast of Chad healthcare facilities are limited and couchers active. 1960 patients were screened and 224 of them were blind due to traditional treatment. Couching causes blindness in most cases. Of the treated patients less than 5 % had a vision of 3/60 or more with aphakic spectacles. Traditional healers continue as long as people are ignorant and scared of modern health care, health centres are inaccessible, and their practices are not controlled by the government. Education, information and adequate eye care services can stop these practices.
