Inleiding: Het meer-niveau perspectief als onderzoeksstrategie
Deze inleiding schetst in het kort de ontwikkeling van het meer-niveau perspectief in sociaal-wetenschappelijk onderzoek naar beleid en praktijk in de gezondheidszorg. Het perspectief biedt een model voor het analyseren van problemen in het functioneren van gezondheidszorg. Het heeft tot doel scherper in beeld te brengen hoe belangen in de gezondheidszorg met elkaar botsen en hoe woorden van betekenis veranderen. Belangenstrijd en spraakverwarring vormen belangrijke obstakels in het uitvoeren van een efficiënte gezondheidszorg.

Multi-level perspective as a research strategy.
The introduction deals briefly with the development of the multi-level perspective in research into policy and practice in health care. It is intended to provide a sharper focus on conflicting interests and changing meanings of words so as to produce a model for analyzing problems arising in health care policy and practice.

Woorden en hun betekenis: Primary Health Care van Genève naar Colombia
Er heeft zich rondom Primary Health Care (PHC) een terminologie ontwikkeld, die voor verschillende interpretaties vatbaar is. De auteur geeft aan, hoe op verschillende niveaus (internationaal, nationaal, provinciaal en lokaal) een aantal van de gebruikelijke termen van betekenis verandert. Zij illustreert dit met een case uit Colombia, waar zij een aantal jaren projectcoördinator was van een door Nederland gefinancierd PHC project binnen de provinciale gezondheidsdienst.

The meaning of words: Primary Health Care from Geneva to Colombia
The terminology which has developed around Primary Health Care is open to many interpretations. The author demonstrates how some of the most commonly used terms change meaning at different levels of social organization (international, national, provincial and local). She illustrates this with an example from Colombia where she worked as a coordinator of a Primary Health Care project at the provincial level.

Primary Health Care in Peru: Een problematisch beleidsinstrument
Degene die als tweede man van de WHO direct betrokken was bij de totstandkoming van de Verklaring van Alma Ata in 1978, kreeg de kans deze in het eigen land Peru in praktijk te brengen. In 1985 lanceerde hij als minister van gezondheid een op papier radicaal Primary Health Care beleid. Het artikel doet verslag van onderzoek naar de oorzaken van de falende uitvoering van dit beleid. Conceptuele tegenstellingen, al dan niet gehuld in versluierend taalgebruik, lijken een belangrijke verklarende factor te zijn. Deze tegenstellingen zijn van invloed op de beleidsformulering en -uitvoering op de verschillende niveaus van het staatsgezondheidszorgsysteem. Het meer-niveau perspectief bleek een nuttig onderzoeks- en analysekader om deze problematiek bloot te leggen. PHC lijkt in essentie een tè politiek (en daardoor controversieel) concept te zijn om te kunnen dienen als beleidsinstrument voor een rechtvaardiger gezondheidszorg.

Primary Health Care in Peru: A problematic policy instrument
The author reports on his research into the failures of health care policy in Peru between 1985 and 1989. Conceptual contradictions and conflicting interests, often concealed in veiled terms, appear to be important factors leading to such failures. The multi-level perspective reveals the existence of such contradictions at different levels of policy formulation and execution. As a concept Primary Health Care appears too political (read: controversial) to be used as a policy instrument for a more equitable distribution of health.

Traditionele genezers in Mozambique in ere hersteld
Tot voor kort werden traditionele praktijken en haar beoefenaars in Mozambique min of meer beschouwd als ongewenst. Hierin is sinds 1989 een duidelijke ommekeer waar te nemen. De auteur die bijna vijf jaar in Mozambique gewerkt heeft vroeg zich regelmatig af waarom er nu wel belangstelling voor samenwerking met traditionele genezers was. Deze vraag is niet bevredigend te beantwoorden vanuit het oogpunt van de volksgezondheid alleen. Welk belang heeft de huidige regering bij aandacht voor cultuur en traditie? Wat verwachten gezondheidswerkers van meer inzicht in lokale opvattingen over ziekte en gezondheid? Welke rol spelen de traditionele genezers zelf? Een benadering vanuit een meer niveau perspectief kan meer inzicht geven in deze complexe ontwikkeling.

The rehabilitation of traditional healers in Mozambique.
Until recently traditional healers and their practices were considered undesirable in Mozambique. Since 1985 their position has clearly been reversed: the author asks how and when this happened. These questions cannot be sufficiently answered from the point of view of public health. Instead one must ask what interest does the government have in restoring traditional ideas and practices? What do health workers expect from a better understanding of local ideas concerning health and illness? What is the role of traditional healers? A multi-level approach helps answer these questions.

Pepo staat voor innerlijke geestkracht of energie, die verbonden is met het goddelijke. De doelstelling van de therapie is om de innerlijke geestkracht te doen ontwaken door een staat van bewustzijnsverandering, zodat een proces van zelfgenezing en spirituele groei op gang wordt gebracht.

Pepo therapy in South-West Tanzania: Singing and trance and the awakening of inner forces
This article on the phenomenon of 'pepo' in sickness and healing is based on research in the Southwestern Highlands of Tanzania. The author discusses the treatment of 'pepo', which proves to involve not only the healer as an intermediary, but also the patient. Through rhythmic singing sessions people enter an inner world. 'Pepo' means inner spiritual force or energy linked to the divine. The aim of this therapy is to awaken the inner spiritual force through an altered state of consciousness, which then sets in motion a process of self-healing and spiritual growth.
